Mentor Form - We Are VB : We Are VB

WE ARE VB’s mentoring program pairs adults in our community with at-risk high school students to meet on a regular basis throughout the school year. Mentoring sessions are typically scheduled twice per month for 50 minutes from September through May. New mentors are asked to attend a brief scheduled training session, hosted by a school social worker, prior to participating in the mentoring program. If you are interested in serving as a WE ARE VB mentor, please complete this application and we will be in contact with you shortly. The information on this form will be kept confidential and will help us pair you with a student. The following are disqualifying criteria for the role of mentor with WE ARE VB:

Prior history of abuse of children, sexual or otherwise
Conviction of any other crime in which children were involved
History of violence or sexually exploitive behavior
Termination from a paid or volunteer position caused by misconduct with a child
Failure to disclose a felony conviction, particularly anything related to violence, abuse or children
